About the Program
After treatment is completed, we provide rehabilitation and ongoing care to help heart patients get well and stay well. Crozer-Keystone’s cardiac rehabilitation program, which is certified by the American Association of Cardiovascular and Pulmonary Rehabilitation, consists of monitored exercise sessions, cardiac risk factor modification, education and counseling. Cardiac rehabilitation services are offered at Springfield Hospital.
To enter cardiac rehabilitation, patients need a physician referral. The length of cardiac rehabilitation varies, depending on a patient’s condition and what his or her health plan allows. Regardless of the amount of time patients spend in the program, our staffs work to make sure that they get as much out of the process as possible. Every patient meets with a staff member before release to review medication and exercise instructions, and to go over what to do if he or she experiences a problem.
Cardiac Transition Program
If our patients have completed cardiac rehabilitation and are not ready for independent exercise, they may participate in a cardiac transition program. This consists of aerobic exercise, strength and endurance training and functional exercise for the tasks of daily living. It helps patients get used to exercising on their own, and shows them how to take charge of their cardiac health. In addition, the Crozer-Keystone Health System makes it even easier to get involved in an independent fitness program through the wellness programs available at the Healthplexâ Sports Club in Springfield.
