Understanding the Role of Home
Health Care Services 

Crozer-Keystone's Home Health Services provide care in the comfort of patients' homes. These patients may be newly discharged from the hospital, referred directly by physicians' offices.

Health care and supportive skilled services are provided through a team approach that incorporates the quality care that has been a CKHS tradition. The Home Health team, a group of highly skilled professionals, works in cooperation with patients' physicians to formulate an individualized plan for care.

 

Will I still see my doctor?

 

Our registered nurses and therapists coordinate your home care services under your physician's guidance. Even though we keep your physician up-to-date on your home care progress, you will continue to make regular office visits.

The Benefits of Home Care

 

"There's no place like home" is a popular saying. Often, patients recover more quickly in the familiar and comfortable surroundings of their own homes.

 

Through the use of a home health care program, patients and their families are assured that a highly-skilled professional staff provides care for individuals or those making the transition from hospital to home.

 

Home health care may shorten or eliminate the need for an inpatient hospital stay or may help the patient delay or avoid a nursing home placement.

  • Pain management 
  • Certified wound specialist
  • Enterostomal therapy (comprehensive wound care, as well as assistance with ostomies and incontinence)

Mother-Baby Connection

 

Our special Mother-Baby Connection provides support for recently discharged mothers and infants, lactation-certified nurses for mothers who choose breastfeeding and phototherapy at home so infants can go home when their mothers do.

How to Receive Services

 

A patient's family member, friend, physician or other health care professional may request home care services. Acceptance is based on approval by the patient's physician and other medical criteria. The CKHS Home Health staff will gladly work with anyone who is interested in home care services to evaluate patient needs and eligibility and formulate an individualized plan of care.

 

Payment for Home Health Services

 

Our home care intake specialists will make all of the necessary arrangements and do a full evaluation of your insurance coverage prior to your first home care visit.

 

No person shall, on the grounds of race, creed, color, sex, sexual preference, national origin, disability or source of payment, be excluded from participation in, be denied benefit of, or otherwise be subjected to discrimination in the provision of any care or service at Delaware County Memorial Hospital.

Home health patients have access to a nurse by phone 24 hours a day, seven days a week

         

Computerization - Our Home Health professionals carry laptop computers to allow them to keep your health records up-to-date and communicate electronically with other members of the Home Health team.

Hospice Services

 

Hospice is care and support for terminally ill people and their loved ones. Hospice philosophy encourages the continuation of a meaningful life in the presence of a terminal illness. The Hospice Program can provide physical, emotional, social and spiritual support to Hospice patients and their families/caregivers.

 

The Hospice Interdisciplinary team consists of nurses, a social worker, clergy, home health aides, volunteers and therapists. Services provided include:         

  • Home visits by nurses, therapists, a social worker, spiritual counselor and volunteers 
  • Home health aides to assist with personal care    
  • Family teaching       
  • Counseling    
  • Assistance with medications, medical equipment and supplies     
  • Assistance of Hospice volunteers for respite, errands, and shopping     
  • Physical, occupational and speech therapists are available as needed    
  • Assistance with community resources       
  • Bereavement services 

The Hospice team coordinates care with the patient's physician to develop an individualized plan for care for symptom management and pain relief.

 

A Hospice nurse is on call 24 hours a day, seven days a week.

 

Hospice neither prolongs life nor hastens death, but helps patients live out the remaining time of their lives in comfort, dignity and peace.

 

Hospice program explanations are available in person at home or in the hospital.
