Each stage of your child’s development brings new wonders, new challenges, and new health and well-being concerns and needs. The Children’s Health Connection Reminder Program, presented by Crozer-Keystone Women and Children’s Health Services, is a free program that addresses these growing concerns. It connects you to the information you need about your child’s health and well being when you need it.
The Children’s Health Connection Reminder Program will help you keep up with good health, safety, nutrition and well-being information needed at important stages in the development of your infant, toddler, school-age child or adolescent.
You will receive free reminder post cards providing you with information to help keep your child(ren) healthy, covering: childhood immunizations, growth and development, nutrition, good health and children’s safety information for children ages 2 months to 18 years old.
You can also take advantage of Crozer-Keystone’s toll-free number, 1-800-CK-HEALTH (1-800-254-3258), to find a physician or click here, and connect with the resources of the Crozer-Keystone Health System - the largest health care provider and employer in Delaware County.
