As a society, we all struggle with talking about end-of-life issues. Often families feel it is too upsetting to ask their loved ones how they would like to be cared for during their final days. Things to Consider:
• How to start the conversation with your loved ones.
• How to use a family occasion to talk to those you care for about their wishes.
• How to encourage the sharing of thoughts and feelings.
• The important topics that need to be addressed.
• Important documents (advance directives, living wills, power of attorney) that should be in place.
• Who should be involved in the conversation.
The Peaceful Journey Partnership is a Crozer-Keystone Health System initiative designed to help individuals live out their final days as they wish.
