Newly eligible employees who enroll before their benefit effective date. If you enroll after your effective date, please submit your paper Enrollment Form.

All benefit eligible employees who are making changes during the annual open enrollment period that is held each fall.
Be sure to have your Personal Identification Number (PIN) before you begin. Your PIN is printed on the Enrollment Form that is included with your Choices benefit booklet, which is mailed to your home by Employee Benefit Services.
