What is Targeted Case Management?
Case management is a direct service designed to ensure that individuals with serious and persistent mental health concerns have access to community services and agencies that will enable them to live in the community in a safe, stable, and healthy manner. These services are regulated by the Department of Public Welfare and Office of Mental Health. Case Management is rooted in the Community Support Program.
What can I expect from my case manager?
Your case manager will partner with you and your support systems toward achieving wellness and the management of symptoms. Your case manager will assist you in setting goals, accessing services to meet those goals and serve as an advocate in your journey.
What types of services are available?
Examples of services are:

Assessments and service planning which is focused on your needs and strengths

Assistance and advocacy with access and use of community resources and services

Problem solving
-Informal support network building
-Supportive contacts in the community
How do I become eligible for these services?
You must meet certain diagnostic and treatment criteria to be eligible. If these are met, your doctor will write an order and you will be assigned to Targeted Case Management.
How long will I be eligible for services?
Services are available for as long as they are needed. The case manager must be able to meet with you in order for services to stay in place. You and your case manager will mutually determine this.
I’ve heard of TCM, RC and ICM. What are these?
Targeted Case Management (TCM) has two levels of care:
