The Dementia-Specific ADC was created for those who suffer from Alzheimer's disease and other related dementias. Led by a gerontologist, this program focuses attention on the individual's remaining strengths while reinforcing a sense of dignity and self-worth.
Medical ADC
The Medical ADC focuses on care, rehabilitation, education and prevention. Led by advance practice nurses, this program's goal is to promote healing and wellness. Our clinicians also act as advocates and liaisons to primary care physicians, health care systems and social service agencies.
Services Provided
While our primary goal is to provide a balanced approach to care for the patient, we also strive to give support, education and peace of mind to the caregiver. With that in mind, we are pleased to offer the following services:
Support Groups
We offer an Alzheimer's Association-affiliated support group that meets on the second Tuesday of each month, from 10 to 11:30 a.m. at the Crozer Silberman Center. Crozer-Keystone Health System offers many other support groups at its five hospitals. Call the Senior Support Line at 1-800-CKHS-KEY for more information.
Education
Understanding an illness, the cause, what to expect, and the outcome can help empower patients and caregivers. Our staff will provide valuable information and resources to help patients and caregivers.
Counseling
Our professional staff are available to help the caregiver understand the changes and emotional impact that a chronic illness has on the entire family.
Respite
The Adult Day Centers provide a break from the 24-hour-a-day responsibility of caring for a loved one, and may also prevent or delay nursing home placement.
A Typical Day
A typical day in the Adult Day Centers may offer any or all of the following activities: music, art, reminiscence and activities that promote self-worth, exercise and discussion groups, meals, and much more.
