Hospice is care and support for terminally ill people and their loved ones. Hospice philosophy encourages the continuation of a meaningful life in the presence of a terminal illness. The Hospice Program can provide physical, emotional, social and spiritual support to Hospice patients and their families/caregivers.
The Hospice Interdisciplinary team consists of nurses, a social worker, clergy, home health aides, volunteer and therapists. Services provided include:

Home visits by nurse, therapists, a social worker, spiritual counselor and volunteers.

Home health aides to assist with personal care

Family teaching

Counseling

Assistance with medications, medical equipment and supplies

Assistance with Hospice volunteers for respite, errands and shopping

Physical, occupational and speech therapists available as needed

Assistance with community resources

Bereavement services
The Hospice team coordinates care with the patient’s physician to develop an individualized plan for care for symptom management and pain relief.
A Hospice nurse is on call 24 hours a day, seven days a week.
Hospice neither prolongs life nor hastens death, but helps patients live out the remaining time of their lives in comfort, dignity and peace.
Hospice program explanations are available in person at home or in the hospital.
