The Center for Diabetes provides outpatient education for individuals with new onset or uncontrolled diabetes mellitus, or for those who desire intensive control. The goal of intensive management is to achieve near-normal glucose levels. Special instruction is available for the individual with gestational diabetes.
The focus of the center is to help you achieve blood glucose control by balancing meals, exercise and medication, when necessary.
The Center for Diabetes has a qualified interdisciplinary team of nurses, registered dietician, a pharmacist, and an exercise physiologist under the guidance of a medical director. Together, this team works with you and your primary care physicians to develop an educational plan to meet your specific needs.
Program participants are required to have a referral form from their physician.
