About the Senior Support Line
 
An elderly man with arthritis is looking for a structured exercise program tailored to his physical needs and medical condition. A working woman, feeling she has nowhere to turn, is looking for a safe place for her father with Alzheimer's disease while she is at work.
 
Fortunately for these two people and many like them across the Delaware Valley, help is as close as their telephones.
 
Many quality programs and services geared toward seniors exist throughout Crozer-Keystone Health System, as well as in our community. Now, through one phone number, patients, their families, and physicians can talk to a trained clinician who will assess their needs, provide information, and refer them to the appropriate senior services. In addition, after referrals have been made, we follow-up with callers. Our clinician remains involved in the process, and collaborates with the appropriate services to ensure a positive outcome for the patient.
 
1-800-CKHS-KEY is available 24 hours a day, seven days a week.
 
Services Provided
Connect to Local and National Resources
With 1-800-CKHS-KEY, one call truly does it all. Connect to Crozer-Keystone programs, such as: the Adult Day Centers, geriatric behavioral health services, the Geriatric Evaluation and Management Program, substance abuse programs and exercise programs for seniors with medical conditions.
 
In addition, we connect our callers to local and national community-based services. Some areas where we provide assistance include: adult day care, nursing home information and/or referrals, health insurance and prescription needs, assisted living, respite care, senior center information and referral, in-home services, medical alert services, transportation, home-delivered meals and much more. Callers can also be directed to resources available through the County Office of Services for the Aging (COSA). Coupled with the above, our Senior Support Line serves as a bridge to services between our inpatient hospitals and community agencies.
